Problem List

The problem list is a key portion of the chart.  It must neatly but succinctly summarize the important aspects of a patient's medical condition.

Here is the Problem List form we will be adopting soon.  It was developed by Terry Ruhl, MD while at Midland Family Practice Residency, then adapted for our use.  You are free to use it.
